3 / Eligibility Review
Schedule an Appointment: Arrange a time for an initial meeting with our team. This can be conducted in person or virtually.
Submit Documentation: Provide necessary documentation such as medical records, psychological evaluations, and any relevant legal documents.
Assessment: Our professional staff will conduct a thorough assessment to understand the individual's medical, psychological, and social needs.
4 / Application Submission
